How to Complete New Patient Forms

Step 1 — Register Online

All new patients must first register through our secure online system. Registration allows us to collect basic demographic information, insurance details, and required forms.

Step 2 — Complete Required Forms

During registration, you will be prompted to complete the following forms:

  • Patient demographics

  • Insurance information

  • Medical history

  • Medication list

  • Consent and acknowledgment forms

Step 3 — Prepare for Your Visit

Please bring the following items to your appointment:

  • Valid photo ID

  • Insurance card (if applicable)

  • List of current medications

  • Any relevant medical records

Arriving prepared helps ensure a smooth check-in process.

Insurance & Billing Information

Lakepointe Primary Care is an insurance-based primary care clinic. Visits are billed through insurance according to your plan’s copays, deductibles, and coinsurance.

Self-pay options are available for uninsured or out-of-network patients.

 

Laboratory services: Laboratory testing is performed by independent third-party laboratory vendors. Lab services are billed separately by the lab. Lakepointe Primary Care does not manage or control laboratory billing or coverage decisions.
