Step 1 - Register With Our Clinic First

Quick online registration to get started

Before scheduling your visit, new patients are required to register with our clinic through our secure online system. This helps us collect your information, verify insurance, and prepare for your appointment.

  • Complete required forms at your convenience

Once registration is complete, our team will guide you through scheduling your visit.

Step 2 — Insurance & Check‑In

We accept most major insurance plans and Medicare

Visits are billed through your insurance according to your plan’s copays, deductibles, and coinsurance. Self‑pay options are available for uninsured or out‑of‑network patients.

What to bring:

  • Valid photo ID

  • Insurance card (if applicable)

  • Medication list

  • Completed new patient forms

Step 3 — Your Appointment

Personalized medical care, not rushed visits

During your visit, you’ll meet with your provider to discuss your concerns, review your health history, and create a care plan tailored to your needs.

Appointments may include:

Step 4 — Labs, Referrals & Follow‑Up

Laboratory testing is performed by independent third-party laboratory vendors. The laboratory bills your insurance directly for their services. Lakepointe Primary Care does not manage, process, or control laboratory billing or coverage determinations.
